HB 4224, Section 181.105 of the Health and Safety Code requires the following information be provided:
- To request your health records from Well Counseling & Consulting, PLLC, you need to
- send an email
- complete a Release of Information (even if the records are for yourself)
- Sydney will get you the records within 14 days and you will be charged the legal amount to be charged per page.
